Dosing Guide
Clear, clinician-aligned dosing information for Mounjaro (tirzepatide)
Key Dosing Principles
Once Weekly
Administered once weekly on the same day. Can be given at any time, with or without food.
Gradual Titration
Most patients start at 2.5 mg and increase by 2.5 mg every 4 weeks to help reduce GI side effects.
Individualized
Maintenance dose is individualized based on response and tolerability. Many people do well below the highest dose.
Standard Titration Schedule
| Period | Dose | Purpose | Notes |
|---|---|---|---|
| Weeks 1-4 | 2.5 mg | Initiation | Typical starting dose |
| Weeks 5-8 | 5 mg | First increase | Monitor for GI side effects |
| Weeks 9-12 | 7.5 mg | Titration | May maintain if response adequate |
| Weeks 13-16 | 10 mg | Dose increase | Often used as maintenance |
| Weeks 17-20 | 12.5 mg | Further increase | If response at 10 mg not adequate |
| Weeks 21+ | 15 mg | Highest dose | Highest labeled dose |

If Your Pen Has Been Frozen
Freezing may affect the medication. If you suspect your pen has been frozen, check the following before use.
Before Use, Check:
- Pen integrity: Ensure the pen/vial has not cracked or been breached. If liquid is leaking, discard.
- Solution clarity: After thawing, inspect the liquid. If it is clear and colorless, it can be used. If you see particles, cloudiness, or discoloration, discard.
Thawing Guidelines
- Thaw at room temperature or in a lukewarm water bath. Some research suggests room temperature thawing may be better than refrigerator thawing.
- Avoid refreezing. Repeated freeze-thaw cycles can affect the medication.
- Use promptly after thawing when possible.
Potential Effects
- Possibly reduced efficacy: Freezing may cause some degradation, which could reduce potency.
- Reduced shelf life: A freeze-thaw cycle may reduce remaining shelf life. When in doubt, consult your pharmacist or the manufacturer.
